Future perspectives and the way ahead

Most substance-abusing adolescents receive expensive day treatment or residential treatment rather than research-based outpatient treatment in their communities. During the last decade, there has been a steady increase in outpatient treatments for adolescent substance use outside traditional clinic, school, drug court, or shelter settings. There has also been a development in which treatment type and approach (e.g., cognitive behavior therapy) rather than treatment model or brand (e.g., MST and FFT) have come into focus. In several countries, practice guidelines in behavioral health care favors on-site treatment over referrals, and behavioral therapists should be prepared to manage common adolescent substance use problems (Hogue et al., 2014). The field of research-tested intervention now represents almost every form of treatment format (individual, group, family) and orientation (behavioral, client-centered, cognitive behavioral, drug counseling, and family systems). Given the richness of the field, Hogue et al. (2014) argue that rather than developing more brand-name models, the efforts should target the scaling-up of evidence-based interventions in large-scale implementation efforts. The authors therefore argue in favor of introducing behavioral interventions for adolescent substance use into various community settings to reach underserved groups. And because it has proven difficult to import research-developed models into community settings with fidelity, there is a great demand for quality assurance methods that can sustain evidence-based approaches in routine care. They also emphasize that even the top treatment models have much room for improvement; the best performing family-based treatment proved to have an ES of d = 0.26 which is quite low (Tanner-Smith et al., 2013).
